You will need to have this information sent to UWG, by providing our Title IV school code of 001601. Or if you prefer to only apply for the HOPE scholarship, you can complete the Georgia HOPE Scholarship and Grant Application Form online, which can be found at www.gacollege411.org.
Submit Immunization Form
A UWG Immunization Form must be completed by a physician or health official and returned prior to enrollment.
