Instructions for Forms: You will need Acrobat Reader to use these forms.  You may type information into certain fields.  However, you must print the form, sign and return it to the Financial Aid Office by US Mail at University of West Georgia, Office of Financial Aid, 115 Aycock Hall, Carrollton, GA  30118; by Fax at 678.839.6422; or in person at the Enrollment Services Center (ESC) in Parker Hall. If delivering the forms in person, please see staff members at the front counter windows.
