Termination of University Employment Notification
For Benefited Employees Only
This notification is required if a benefited employee is leaving the employment of the University. Please do not use this form for student employees or casual labor employees. Please fill out this form so the appropriate personnel are informed in advance of a change in employment status. For security, coordination, and informational purposes; it is best if the appropriate personnel across the university are informed as soon as possible of the impending departure of an employee.
Budget Services and Human Resources will still require a PAR to be completed and submitted as before.
