Documentation required for service contracts and POs
If you are responsible for coordinating service contracts of any type please read and save this notice. Effective July 1, 2013, Georgia law requires that for any service contract $2,500 or greater UWG is are required to obtain a signed and notarized Immigration and Security affidavit from the supplier prior to entering into the contract. Subcontractors, if any, are also required to submit the affidavit. Suppliers must verify legal status of their workforce through use of the Federal E-Verify system. If you do not obtain this form prior to submitting your request for a contract or PO your request will be delayed until Purchasing staff can secure the form.
