Dependent Form

As part of the Shared Services program, ADP (Automatic Data Processing), in contract with insurance vendors, will be required to have valid social security numbers for all employee dependents (spouses and children).

Please complete the necessary form, print it, and then return it to Human Resources as soon as possible.
