Note: The survey below is only a "COPY" of the scantron survey which is handed out and mailed to each video-conferencing (GSAMS) site for each UWG distance course. When surveys are mailed, self-addresses stamped envelopes and #2 penciles are included for the students to use. Results are anonymously computed by the UWG Instructional Technology Department, stored in a database, and forwarded to the Distance and Distributed Ed Center for distribution to faculty and department chairs.
