This scholarship is LIMITED to only one Learning Festival course for Summer 2008 and only forone of the specified courses:  Science Technology, Science Pedagogy for Middle and High School,Science and Health, Differentiated Instruction, and Teaching of English Language Learners. We have a limited number of these scholarships, so students will receive them on a first-come, first–served basis.

DIRECTIONS: :

• Before emailing this completed form, you MUST obtain permission from your academic advisor to receive credit for this elective.

• Once advisor’s permission is granted, email this form to Ros Duplechain (rduplech@westga.edu), Ms. Thomas-Hill (cthill@westga.edu), AND Ms. Patricia Wells (pwells@westga.edu). They will see that you are registered for your chosen Learning Festival course and designated as a recipient for the Learning Festival scholarship.

• Follow up with a phone call to Ms. Thomas Hill or to Ms. Patricia Wells (768-839-6419) ASAP. They need to ensure that UWG has all of your necessary paper work.
