Everyone must submit a Graduate Student
Application, Admission to Candidacy Form, the appropriate Proposed Plan of
Study, and a Graduate Information Form to apply for graduation. Please
print the forms listed under the degree heading which pertain to your
program. For example, if you are graduating from a M.A. program, please
print the forms listed under the Masters degree heading.

These
forms are for MA, MS, MSRP, MBA, MPACC, MM, MPA, & MSN
Candidates.
Please
note that candidates for the following degrees will use the Non-Thesis
forms: MBA, MPACC, MPA, and MSRP.
Other degree programs have a thesis or non-thesis option (e.g.,
Applied Computer Science, Biology, English, History, Music, Nursing,
Psychology, and Sociology). Therefore, unless you are completing
a thesis as a requirement for completion of your degree, you will also
use the Non-Thesis form.
