Why do I need this?

Accepting/Declining Student Loans This document will explain how to accept or decline your financial aid award and also give you further instructions about how to apply for a Stafford Loan.
Appeal Form Submit this form if you are appealing Pace of Progression, GPA, and/or Maximum Time Frame Suspension.

Asset Information

Submit only if requested. When you completed the Free Application for Federal Student Aid (FAFSA) you either failed to answer the questions concerning yours and/or your parents’ assets OR the assets you reported do not seem to support the earned interest, dividends on your tax return, or income reported on Schedule E of your 2014 1040 tax return. This information must be corrected before your application can be processed.

Child Support Paid Submit only if requested. You indicated on your FAFSA that someone in the household paid child support during the prior calendar year.
Dependent Income Reduction Form Submit if you or your family experienced significant changes to your financial situation, such as loss of employment. Along with this form, you must also submit a Verification Form and Income Summary Form.
Dependent Untaxed Income Submit only if requested. Report all sources of untaxed income within the prior calendar year, as required by the U.S. Department of Education.
Document Release Form Submit this form to request copies of financial aid documents you have submitted to UWG or certain notices you have received from Financial Aid.
Federal Work Study Eligibility/Cancellation Submit this form to apply for Federal Work Study if you did NOT indicate interest on the FAFSA or submit this form to cancel your Federal Work Study award.
Financial Statement Form Submit this open-ended form to provide the Financial Aid Office with additional information for clarification of outstanding financial aid matters.
High School Completion Submit only if requested. The U.S. Department of Education has requested verification of your high school completion.
Household Expenses Submit only if requested. The income reported on your FAFSA appears insufficient to support your household size. Independent students and parents of dependent students are required to complete all sections of this form to clarify how your living expenses are currently being met.
Identity and Statement of Educational Purpose Submit only if requested. The U.S. Department of Education has requested verification of your identity and statement of educational purpose. The form must be completed in the presence of the Financial Aid Office.
Independent Income Reduction Form Submit if you experienced significant changes to your financial situation, such as loss of employment. Along with this form, you must also submit a Verification Form and Income Summary Form.
Independent Untaxed Income Submit only if requested. Report all sources if untaxed income within the prior calendar year, as required by the U.S. Department of Education.
IRS Data Retrieval Tool This document will explain how to access the IRS Data Retrieval Toom to transfer your income tax information to the FAFSA.
Loan Change Request Form This form can be submitted if you need to request a change/cancellation of your loan.
Marital Status Submit only if requested. You either indicated on the FAFSA that you are married or included a spouse in the Household Chart of the Verification while using the filing status of "Head of Household/Single" on taxes for 2014. This conflicting information must be resolved before awarding any financial aid.
Missing Parent Information Submit only if requested. You did not complete the FAFSA with parental information. It will explain the various options you have and give you additional instructions.
Missing Signature Submit only if requested. The FAFSA was not signed during the initial application process.
Parent PLUS Loan Instructions  This document will explain how to apply for a Parent PLUS Loan.
Selective Service Requirement Submit only if requested. Verify registration with the Selective Service.
SNAP Benefits Received Submit only if requested. You indicated on your FAFSA that someone in the household received Supplement Nutrition Assistance Program (SNAP) benefits during the prior calendar year.
Student Dependency Status Worksheet Submit only if requested. Confirm your dependency status; this form will provide instructions concerning additional documentation required.
Third Party Release Form Submit this form to give the Financial Aid Office permission to discuss your financial aid with a third party.
Verification Form Submit only if requested. Your FAFSA was selected for standard verification.
