The form is open from April 1 – July 1 to request Fall/Spring/Summer semesters. The form is open from Oct. 1 - Jan. 1 for spring start students to request Spring/Summer/Fall semesters. Students may request up to three semesters total. Students must complete a new form each year. This is a merit-based award, and eligible students are encouraged to submit this form as early as possible. Students enrolled in online graduate programs are not eligible.
