Please only use this form for Argos Reporting requests and issues. Banner Request should be submitted to the Banner Request/Issue Form. All other IT-related requests should be sent to servicedesk@westga.edu.

Please review and complete the Banner Authorization Release and Confidentiality Agreement forms located on the Agreement Forms page and file it with your supervisor.
