Please only use this form for Argos Reporting requests and issues. Banner Requests should be submitted to the Banner Request Form. All other IT-related requests should be created via the Self Service Portal or by contacting the Service Desk at 678-839-6587.
Please review and complete the Banner Authorization Release and Confidentiality Agreement forms located on the Agreement Forms page and file it with your supervisor.
