Accepting/Declining Student Loans This document will explain how to accept or decline your financial aid award and also give you further instructions about how to apply for a Stafford Loan.
Appeal Instructions This document will explain how to submit a Financial Aid SAP Appeal for Pace of Progression, GPA, and/or Maximum Time Frame Suspension.
Document Release Form Submit this form to request copies of financial aid documents you have submitted to UWG or certain notices you have received from Financial Aid.
Federal Work Study Eligibility/Cancellation Submit this form to apply for Federal Work Study if you did NOT indicate interest on the FAFSA or submit this form to cancel your Federal Work Study award.
Financial Statement Form Submit this open-ended form to provide the Financial Aid Office with additional information for clarification of outstanding financial aid matters.
IRS Document Request Instructions This document will explain how to request IRS tax information and provide steps to use the IRS Data Retrieval Tool to transfer your income tax information to the FAFSA.
Loan Change Request  This form will provide instructions on how to submit a Loan Change Requset Form, if you need to request a change/cancellation of your loan.
Missing Signature Submit only if requested. The FAFSA was not signed during the initial application process.
Parent PLUS Loan Instructions This document will explain how to apply for a Parent PLUS Loan.
Parent PLUS Loan Intent Form  Submit only if requested. This form is to determine a parent's intent as a Parent PLUS Loan borrower.
Parent PLUS Loan Mismatch Form Submit only if requested. This form is to verify that a Parent PLUS applicant is an eligible Parent PLUS borrower (biological or adoptive parents only; a step-parent if currently married to biological parent).
