Termination and Transfer Notification Form This notification is required when a faculty, staff, or casual labor employee is leaving the employment of the University, or is transferring departments on campus. Please fill out this form so the appropriate personnel are informed in advance of a change in employment status. For security, coordination, and informational purposes; it is best if the appropriate personnel across the university are informed as soon as possible of the impending departure of an employee.
