Termination of University Employment Notification Form

This notification is required when a faculty, staff, or casual labor employee is leaving the employment of the University, or is transferring departments on campus. Please fill out this form so the appropriate personnel are informed in advance of a change in employment status. For security, coordination, and informational purposes; it is best if the appropriate personnel across the university are informed as soon as possible of the impending departure of an employee.

Budget Services and Human Resources will still require a PAR to be completed and submitted as before.

Does employee meet USG Board of Regents criteria for retirement?
Does employee wish to receive information from the UWG Association of Retired Faculty & Staff? (re: activities, healthcare benefits, etc.)
