Intent to Submit Form

This form serves as a letter of intent for grant submissions. Please complete this form at least one month prior to the submission deadline, and a member from ORSP will be in touch with you to walk you through the application process.

Proposal Routing Form (PRF)

Proposals will not be submitted until required Proposal Routing documents are completed and contain all required approvals.

Proposals will not be submitted until all required documents are completed and have all required approvals. It is the responsibility of the Principal Investigator/Project Director to ensure all documents are approved and submitted to ORSP 5 business days before the sponsor deadline.

All individuals engaged in research involving human participants must complete an educational program related to the responsible conduct of research prior to initiation of the project. Please visit the CITI Program page for more information.

Financial Conflict of Interest Disclosure Form

If you indicated that you do have a significant financial interest when completing the Proposal Routing Form, you must complete a Financial Conflict of Interest Disclosure Form before you can submit your proposal.

Cost Share Request Form

If your project proposes cost sharing, then you must complete the Cost Share Request Form.
